Frequently Asked Questions

Who is Choice Health for?

Older adults living with ongoing health conditions like diabetes, heart disease, or COPD who want help staying on track at home. We also help patients who have just come home from a hospital stay.

What services do you offer?

Chronic care management, remote patient monitoring, follow-up after a hospital stay, and telehealth visits with our providers.

Do I have to give up my own doctor?

No. We work alongside your primary care provider, your specialists, and your home health agency, and we keep them informed.

Do you come to my home?

Our visits are done by telehealth, so you can see your provider without leaving home.

How does remote patient monitoring work?

You use a connected device at home, such as a blood pressure cuff, and your readings are sent to our team. We review them regularly and reach out when something needs attention.

Is someone available around the clock?

No. Our team is available during business hours. Remote monitoring is not an emergency service. If you have an emergency, call 911.

What does it cost?

Medicare and many other insurance plans cover these services. Depending on your plan, you may owe a copay or coinsurance. Before you start, we'll check your coverage and explain what, if anything, you'll owe.

Where do you serve patients?

We currently serve patients in Texas and New Mexico. Give us a call to confirm we can serve you.
